We provide life enriching Housing Stabilization Services that help individuals find and keep housing.
Eligibility
You can get Housing Services if you:

- Have Medical Assistance
- Are 18 or older
- Have a disability or disabling condition
Contact Us Today
- Are in one of the following situations:
- Homeless or at risk of homelessness
- Moving out of an institution nursing facility, or certain other group settings,
- At risk of institutionalization.
- Are assessed to need help with at least one of these areas:
- Communication
- Mobility
- Decision-making, or
- Managing moods or behaviors
